Core Responsibilities
1. Medication Verification & Dispensing: Clinical pharmacists review every doctor's prescription for correct dosing, drug compatibility, patient allergies, and organ function (such as renal or hepatic impairment) before releasing medications.
2. Clinical Consultation: Pharmacists actively participate in patient rounds—especially in Intensive Care Units (ICU), Oncology, and Emergency Departments—to recommend optimal drug regimens and adjust antibiotic or chemotherapy dosing.
3. Aseptic & Sterile Compounding: Dedicated cleanroom personnel prepare intravenous (IV) solutions, Total Parenteral Nutrition (TPN), and hazardous chemotherapy infusions under sterile laminar flow hoods.
4.Inventory & Cold Chain Management: Pharmacists oversee high-value pharmaceutical supplies, strict narcotic controls, and temperature-sensitive items like insulin and vaccines.
5.Patient Education & Discharge Counseling: Outpatient pharmacists instruct patients on proper administration technique, potential side effects, and drug-food interactions to prevent readmission.
